  Worker doused with chemicals at BP refinery
  “Employee says he was trying to unstop a drain at the PS3B Unit on the premises when other workers resumed cleaning the area without notifying the employee or checking to see if he was still working in the way. As a result, the employee was doused with chemicals of an unknown origin and the employee suffered severe injuries to his eyes, face, and skin. He sought medical treatment at the refinery's clinic and was discouraged from seeking additional treatment directly or indirectly from the employees of BP. Repcon, a contractor, and BP accused of being ultimately faulted for operating the refinery with institutional ignorance or defiance to a culture of safety and accountability, managerial acceptance or encouragement of normalized deviation from appropriate refining procedures, and failing to execute proper safety procedures.”
  Questions:
  1. Could some sort of LOTO type of procedure been put in place?
  2. Do you think someone in middle management was trying to cover their
  behind?
  3. How bad do you think this has hurt the organizations safety culture?
  4. What needs to be done so this is not likely to occur again?
